Intake Form
If you would like to refer someone to McCallum or enquire about becoming a new participant, please complete the New Participant Referral Enquiry Form below. Once your form is submitted, a member of our Customer Experience Team will contact you to discuss your enquiry and guide you through the next steps. If you need support to complete the form, you can call 1800 316 714 and our team will assist you. To make the process easier, it helps to have the participant’s NDIS number and plan details ready before you begin.

Submitting Anonymous Feedback
If you feel your complaint has not been satisfactorily handled or you don't feel comfortable raising a complaint directly with McCallum you can submit feedback or complaints anonymously via our Protected Disclosure process.
- Your feedback, issues and concerns are extremely important to us. It helps us improve as an organisation and implement changes to provide higher levels of service to our customers.
